I've first encountered this just now and couldn't find anything about it. It's an output from an npc_apache on bm_c2a5b. I have no idea what it means.

I would guess that this exists because when beaten, the helicopter enemy will first need to find a place to crash before moving towards that place and actually exploding for real. One place where this might be used is in the helicopter fight at the dam in the beginning of Surface Tension, where the helicopter flies away at zero health instead of exploding.

An Apache helicopter can't really die in Black Mesa, even it is below 0 health. Instead it starts bleeping and smoking.
The OnWhishToDie Output is fired when beeing below 100 health. In game it is used to
a) Fly the helicopter away (at the hydrodam for example) or
b) to crash (after you got the RPG).
Not sure what it does in bm_c2a5b... May be the helicopter is just copied from bm_c2a5a (including I/O).
